Building on these concepts, NAMM has assembled a PCMH team to work with your practice to provide advanced management of patients that demonstrate a large care gap; these patients are at risk for developing significant health problems due to inadequate care for their chronic medical conditions. Our case management staff will work with the PCMH team to perform sophisticated patient outreach. We will ensure the patient understands what their care needs are, help them to overcome barriers to accessing necessary health care services and improve compliance with the physician’s treatment plan. We will work with your team to put an infrastructure in place that delivers optimal, preventative and coordinated patient care that ultimately reduces or eliminates care gaps and improves patient compliance.
