RN or LPN Care Manager for New Medical Home Program

Consider this outstanding opportunity to work at the leading edge of the health care industry, as a telephonic DM/CM within an outpatient ambulatory setting. Under the direction of a primary-care physician, you will coordinate services through the continuum of care.

Utilization Management Nurse

In this full-time position, you will work closely with physicians and hospital staff to coordinate care and benefits for members. You will have 3+ years of recent clinical experience, including an insurance background, DC planning as well as knowledge of Milliman® Care Guidelines. The ability to travel locally is required.
